It was a direct quote from Jesus from about 27AD (assuming a birth in 4BC).

Removed quote of the entire immediately preceding post as redundant.

Name of your church: The Uniting Church, North Belconnen.
We usually say these words:

Our Father in heaven,
hallowed be your name.
Your Kingdom come,
your will be done,
on earth as in heaven
Give us today our daily bread.
Forgive us our sins,
as we forgive those who sin against us.
Lead us not into temptation,
but deliver us from evil.
For the kingdom,
the power and the glory are yours.
Now and for ever.
Amen.

Variations, according to translation into modern English mean no "art", "thy", "thine", as in the Elizabethan English of the King James bible - but the meaning is the same as the traditional version.
Similarly, some say "trespasses"/ "debts" instead of the "sins" used above - same intent. A variation in English usage, not a different prayer.

If you just google search Lord's Prayer, you will be able to find various translations into many languages.
